Complete the enrollment form below to register for any class listed below. Class fee must be paid in full no later than one week prior to the class start date. A $25.00 cancellation fee will be charged if cancellation is received within 48 hour prior to the class start day. Any Cancellations received after 48 hours of the class start day will result in you being charged one half (1/2) of the class fee.
